We have just started to accept credit cards. In order
to prevent fraud and charge backs we will need the following
information.

You will need to fill out our credit card authorization form.

the office at 678-382-3701 and asked it be faxed to you.

Photo copy of drivers License for verification of
our records.

Photo copy of your credit card front and back. Card has to have
a signature on the back panel.

Completed order form.
(please specify what you are ordering...eg...chrome curved
shower rod with pink bows)
All items have to be either faxed to us at
678-382-3710 or mailed to us so we can process your order. We will
not accept any orders if the driver's license does not match the
name on the card or signature differs from the signature on the
back of the card.

It will appear as Atlanta Baths, Kitchen & Tile on
your credit card statement. Your printed receipt will be
mailed to you without 3-5 business days. There are no returns on
special order item that have to be custom made. On all others
returns there is a 35% restocking fee if it is in good condition
in its original box. Shipping will be paid by the customer.
